FAQ

What does my child need to be cleared for sports?

The student

must have a current physical on file with the nurse on the district physical form
must have a current insurance card with their name on it on file with the athletic director
must have a consent form signed by the student and the parent on file with the athletic director

Why must my child have a copy of an insurance card with their name on it?

Proof of insurance must be provided showing that the student is covered. Options to show coverage include:

an insurance card with the student's name on it
an insurance card with the parents name and it shows family coverage (parent name on card must match the student's parent)
a letter from the insurance company showing proof of insurance with the insurance subscriber ID
an EOB (explanation of benefits) from the insurance company showing the students coverage
