All student-athlete clearances must be completed online.
Please see the links below for registration, directions to complete registration, and YouTube video (if desired) for more information regarding student-athlete clearances. If you do not have Internet access you may contact the school to use a school computer to register. After completing the process an email will be sent to the email address that was used to register explaining the next steps in the clearance process.
Clearance will not be granted until the items below are completed and placed in the locked athletic box in the main office (confirmation form and student insurance card may be uploaded). Remember that even if students are cleared to participate by turning in all paperwork, a 2.0 GPA and 20 units passed on the previous report card is required; less than a 2.0 or 20 credits will cause the student to be ineligible to participate until the next official grading period. A student is only cleared when you receive an email stating they are cleared. If you have questions please contact your coach as they have access to see what is missing.
Items that need to be completed and turned in before clearance is granted:
- Confirmation form (online or placed in the athletic lock box; one for each sport)
- Student Insurance Card or proof of coverage (online or placed in the athletic lock box)
- Physical Form (must be turned in directly to the nurse or the athletic lock box; scanned copy will not be permitted).
PLEASE MAKE SURE TO MAKE A COPY OF EVERYTHING THAT YOU PHYSICALLY TURN IN.
