All student-athlete clearances must be completed online.
Please see the links below for registration, directions to complete registration, and YouTube video (if desired) for more information regarding student-athlete clearances. If you do not have Internet access you may contact the school to use a school computer to register. After completing the process an email will be sent to the email address that was used to register explaining the next steps in the clearance process. Clearance will not be granted until the items below are either given directly the Athletic Director, Mr. Sheriff
at a per-arranged time, placed in the locked athletic box in the main office, or uploaded during the registration process (best option). Remember that even if students are cleared to participate by turning in all paperwork, a 2.0 GPA is required and less than a 2.0 may cause the student to not be eligible to participate until the next official grading period.
- Signed Confirmation Form - will be in your email after you complete registration or can be printed from athelticclearance.com
- Physical Exam - you can download a copy of the form from the clearance website and fill out both sides (parent and Dr.). Remember to have the doctor sign date and stamp the physical.
- Copy of Insurance Card - may be uploaded online but MUST have student's name on the card or state family coverage (an Explanation of Benefits may also be used in conjunction with the parent insurance card for verification purposes).
