Transcripts, Diplomas & Records Requests

Please note: Same-day service is not available.

 

The Los Angeles Unified School District’s Student Records Center (SRC) is responsible for the permanent archiving of student cumulative records of former LAUSD students.
 


Graduates: 2014 – Present

Students should email Mrs. Rodriguez to request transcripts.

  • Full name while attending Chatsworth Charter High School

  • Date of birth

  • Year of graduation

  • Mailing address (college/university or home address, if requesting delivery)


Graduates: 1991 – 2013

Students may order transcripts and/or diplomas online through LAUSD:

Graduates: 1990 or Earlier
  1. Email the registrar to request your student record/index number:

    Include:

    • Full name while attending Chatsworth Charter High School

    • Date of birth

    • Year of graduation

  2. Once you receive your record number, visit:

    and select “Former Graduate” to request transcripts and/or diplomas.


Diplomas

  • Chatsworth Charter High School does not keep copies of diplomas.

  • Diplomas must be ordered online at:

  • Graduates prior to 1990 must first request their student record/index number by emailing the registrar.


Third-Party Education / Graduation Verifications
