Apply

There are two ways to apply, electronically and physically. Either way will have a components of paperwork needed to be turned in.  Complete the electronic version through the parent portal and then the form. For a physical form you can print one out or come into the attendance office to pick one up.

General School Apply through the Parent Authentication link, choose Affiliated Charter, complete application and then this General Form
School for Advanced Studies* Apply through the Parent Authentication link, choose Affiliated Charter, complete application and then this SAS Form
GSTEAM Magnet Apply through the Parent Authentication link, choose Magnet, complete application and then this Magnet Form
* Because CCHS is an Affiliated Charter we are not under SAS on district site.
* We are recognized by California as a Distinguished SAS Site. 

 

On Campus

Please consider continuing to wear a surgical face coverings, maintain physical distancing of six (6) feet, and bring your own pen (black/blue) although one will be provided upon request. There is limited space in the office, please only bring essential members of your family to complete the enrollment process;

In order to expedite the registration of your student please complete the following:
 

PDF GUIDE for NEW-TO-DISTRICT Students that are selected applicants to a Choices programs

(Parent/Guardians must have applied online, and accepted a placement prior to pre-enrollment)

 

Choices programs include Magnet, Permits With Transportation, Dual Language Education, Schools for Advanced Studies, Admission Criteria Schools, Affiliated Charters; Open Enrollment; and Zones of Choice online applicants who have received a school assignment.

 

Note: parent must have applied online, and accepted a placement prior to pre-enrollment.

Step 1:
Complete Pre-Enrollment Forms (can be found at:)
The Pre-Enrollment Portal   AND Complete the Following: 
(Copies available in the Administration Building, room 11, Attendance Office, enter through the Vintage St. and Lurline Ave. parking lot. ) 
 
Step 2:
Provide Specific Documents:
i. Age Verification (one of the following) 
      • Birth certificate, Baptismal certificate, Passport, Court order 
      • Health office/vital statistics record of birth certificate date
      • Department of Public Social Services (DPSS) letter verifying birthdate and an explanation of how this was verified 
 
ii. Address Verification (one of the following)
      • Utility service contract, bills or payment receipts (Gas, Water, or Electricity)
      • Property taxes, rental or lease agreement, current rental receipt with address of property on receipt
      • Official government mail (CalWORKS, Social Security, Medi-Cal)
      • Current pay stub, Voter registration
      • NOT accept are telephone, credit card or internet bills
 
iii. Parent/Guardian/Educational Rights Holder/Caregiver (“Parent”) Identification - Student’s birth certificate or baptismal certificate or Court order establishing the parent/guardian relationship AND Adult’s government-issued photo identification (Driver’s license or DMV identification card)
 
iv. Immunization Record - For your reference, the Parents’ Guide to Immunizations Required for School Entry is available at here: https://eziz.org/assets/docs/IMM-222School.pdf
 
v. Transcripts or recent grade reports - Courses taken and grade achieved
 
vi. Students Living with an adult other than Parent - Legal Guardianship Papers
 
vii. If available/applicable you may also submit copies of:
  • Copy of most recent Individual Educational Plan (IEP) or Section 504 Plan
  • Department of Children and Family Services (DCFS) 1399 Form or minute order
  • Department of Probation placement information and/or Juvenile Court minute order
  • Court orders regarding legal custody, physically custody, or restricted contact with the school or child
 
Step 3:
Submit by Completing
The Pre-Enrollment Forms AND Verification Documents AND Come to the Office

Parents of continuing LAUSD students can upload select enrollment forms in their Parent Portal account.  A parent guide can be found here:
