LAUSD and CCHS are continuing precautions to ensure the health and safety of all employees & visitors. Please continue to follow these guidelines in accordance to district policy:

  • Wear a surgical face coverings; (encouraged not mandated)
  • Maintain physical distancing of six (6) feet; (encouraged and recommended)
  • We recommend you bring your own pen (black/blue) although one will be provided upon request;
  • There is limited space in the office, please only bring essential members of your family to complete the enrollment process;
 
In order to expedite the registration of your student please complete the following:
 
Step 1:
Complete Pre-Enrollment Forms (can be found at:)
The Pre-Enrollment Portal   AND Complete the Following: 
(Copies available in the Administration Building, room 11, Attendance Office, enter through the Vintage St. and Lurline Ave. parking lot. ) 
 
Step 2:
Provide Specific Documents:
i. Age Verification (one of the following) 
      • Birth certificate, Baptismal certificate, Passport, Court order 
      • Health office/vital statistics record of birth certificate date
      • Department of Public Social Services (DPSS) letter verifying birthdate and an explanation of how this was verified 
 
ii. Address Verification (one of the following)
      • Utility service contract, bills or payment receipts (Gas, Water, or Electricity)
      • Property taxes, rental or lease agreement, current rental receipt with address of property on receipt
      • Official government mail (CalWORKS, Social Security, Medi-Cal)
      • Current pay stub, Voter registration
      • NOT accept are telephone, credit card or internet bills
 
iii. Parent/Guardian/Educational Rights Holder/Caregiver (“Parent”) Identification - Student’s birth certificate or baptismal certificate or Court order establishing the parent/guardian relationship AND Adult’s government-issued photo identification (Driver’s license or DMV identification card)
 
iv. Immunization Record - For your reference, the Parents’ Guide to Immunizations Required for School Entry is available at here: https://eziz.org/assets/docs/IMM-222School.pdf
 
v. Transcripts or recent grade reports - Courses taken and grade achieved
 
vi. Students Living with an adult other than Parent - Legal Guardianship Papers
 
vii. If available/applicable you may also submit copies of:
  • Copy of most recent Individual Educational Plan (IEP) or Section 504 Plan
  • Department of Children and Family Services (DCFS) 1399 Form or minute order
  • Department of Probation placement information and/or Juvenile Court minute order
  • Court orders regarding legal custody, physically custody, or restricted contact with the school or child
 
Step 3:
Submit by Completing
The Pre-Enrollment Forms AND Verification Documents AND Come to the Office 
 
 
Step 4:
Parent Portal Account Sync to Student(s)

PDF GUIDE for NEW-TO-DISTRICT Students that are selected applicants to a Choices programs

(Parent/Guardians must have applied online, and accepted a placement prior to pre-enrollment)

 

Choices programs include Magnet, Permits With Transportation, Dual Language Education, Schools for Advanced Studies, Admission Criteria Schools, Affiliated Charters; Open Enrollment; and Zones of Choice online applicants who have received a school assignment.

 

Note: parent must have applied online, and accepted a placement prior to pre-enrollment.
